If you are a rep and wish to provide labels to one of your artists for his/her mailing, we require you to submit a signed agreement to ADBASE. Please download the PDF Agreement form, fill in your name and the name of your artist at the top, print it out and have both of you sign the form. Fax the completed document back to ADBASE at 416-921-1025 before the labels have been given to the artist.   Download the Transferring Labels Agreement
